There is no single packing list that applies to every clinic or hospital. Check the facility’s current website or call ahead, especially for a first visit, a referral-based appointment, or an after-hours visit.

A document that works at one facility may not be enough at another. Keep the list below as a preparation prompt, not a guarantee of admission or payment.

• The health-insurance eligibility document or My Number card accepted by the facility. MHLW explains that a My Number card can be used as a health-insurance card at participating facilities; confirm the facility’s actual setup.
• A referral letter or other documents the facility requested.
• Identification, such as a residence card or passport, when the facility asks for it or when it is relevant to your situation.
• A payment method accepted by that facility. Ask before the visit because payment options vary.
• A short note with questions, medication names, or records you want to show. Do not email health information to FMD.

Save the facility’s address and phone number, check its current hours, and confirm whether you need an appointment. If you need language support, ask what is actually available on the day and whether you should bring an interpreter or a translated note.
